Information regarding a person’s Retirement Survivors Disability Insurance or Supplemental Security Income benefits is contained on the form SSA-2458, Report of Confidential Social Security Benefit Information. The Social Security Administration will normally send it to the beneficiary upon request, but it can also be requested by a third party.
To verify RSDI or SSI benefits, taxpayers must send Form SSA-2458 to the appropriate agency. The form is no longer downloadable from the Social Security Administration website as of 2016. Anyone in need of a copy of a form that isn’t available on the website can get one by contacting their regional office or by calling 800-772-1213.
