Form SSA-2458, Report of Confidential Social Security Benefit Information, has information about a person’s Retirement, Survivors, and Disability Insurance or Supplemental Security Income benefits. It is usually sent to the person getting the benefit when they ask for it, but a third party can also ask the Social Security Administration for it.
Form SSA-2458 needs to be sent by taxpayers to any office that needs proof of RSDI or SSI benefits. As of 2016, you can’t download the form from the website of the Social Security Administration. If a person needs a copy of a form that isn’t on the website, they can go to their local office or call 800-772-1213.
